Screening Form
To set up a date with me I require all clients to be screened upon contacting me. This is purely for my safety and nothing more. I will never contact you unless permitted and I strive to keep the utmost client/provider discretion. After I verify you I will I delete all of the information. Again I only use this information to ensure my safety. I truly thank you for understanding. If you decide to not fill out the form or only give me partial information unfortunately we will not be setting up a meeting. If you cannot submit screening through my site contact me and you can email/text the information.
*Non Invasive form of screening: If you are a well known person of status and cannot go through this screening process please email me. To be screened like this you must have a wiki page about you, and a verified twitter account. I will require a dm from this account. I will also be requesting a selfie proving that you are who you say. If a NDA will need to be signed please let me know so we can arrange it. (there is no "celebrity discount")
